Operational Audits
The operational reviews featured here are independent, CYCP-informed community reviews. They are not commissioned, controlled, approved, or endorsed by the organizations being reviewed unless explicitly stated.
The purpose of an independent review is to examine whether an organization’s public commitments to safety, accessibility, equity, inclusion, safeguarding, program quality, and community trust are reflected in observable practice, available records, prior communications, stakeholder experience, and front-line conditions.
An organization does not need to authorize an independent review in order for that review to exist.
These reviews are not presented as internal audits, legal findings, regulatory determinations, or formal investigations on behalf of the organization. They are independent advocacy and community-accountability documents based on information available to Jai Ari Youth & Community Consulting of Six Rivers.
Organizations named in a review are welcome to provide factual corrections, records, policies, responses, or evidence of corrective action. Where appropriate, updates may be added to reflect new information.

A CYCP-informed review of patient experience, communication, privacy, safety, accessibility, discharge/navigation support, and institutional accountability within a community hospital setting. This review examines whether public commitments to exceptional health care and healthier communities are reflected in front-line practice, patient communication, documentation, privacy boundaries, and follow-up processes. Cambridge Memorial Hospital describes itself as a full-service acute care hospital serving Cambridge, North Dumfries, and the Region of Waterloo.

CYCP-informed review of housing, mental-health support, crisis response, communication, and client-centred accountability within a community support organization serving people navigating mental health conditions, housing instability, and system barriers. This review examines whether public commitments to dignity, stability, support, and community care are reflected in practice, access, documentation, and lived experience. Thresholds describes its work as opening doors to mental health and housing, including affordable housing and flexible services across Dufferin-Caledon, Guelph-Wellington, and Waterloo Region.

Summary
An operational review examining whether a legacy youth-serving brand’s public commitments to safety, accessibility, equity, inclusion, safeguarding, and community trust are reflected in front-line practice. This audit was offered as a gratuity at no cost to the association, to highlight concerns that have existed for decades, despite the association’s assertion that they have measures in place.
To date I have conducted two operational audits in-person, which has given me enough information to determine the operational failures at play.
This morning, the CEO of the organization responded to my campaign asking for resolution by attempting to surpress communication of a minority. While the contents were private and confidential, I can say that it is becoming an ongoing theme that the people at the top merely skim concerns, as evident to the left.
While I can’t show the email itself, you are invited to view my response.
The CEO’s response demonstrated an attempt to control the narrative, and preside over other matters that have nothing to do with the association. They attempted to prevent both discussion of the concerns, and the escalation of matters outside of the organization, when they clearly fail to resolve these concerns in house, or without involvement of third parties.
This is harmful to Freedom Of Expression and the rights of rights auditors, vulnerable people and freedom fighters of all backgrounds, especially minorities across our country.
